We are researching the Special Supplemnetal Nutrition Program for Women, Infants and Children (WIC) and want to hear your feedback.

SURVEY PARTICIPATION ONLY TAKES 15-20 MINUTES!

The survey will ask about your experience with WIC clinics, grocery shopping and the food package.

YOU ARE  ELIGIBLE FOR THE SURVEY IF:

• You are currently using WIC benefits.
• You have used WIC benefits in the past 3 years.
• You are currently eligble for WIC but choose not to use it. 

* Must be 18 or older and live in Missouri

Survey responses will be kept confidential.

Those who complete the survey will have the option to enter themselves into a raffle for a chance to win a $25 VISA gift card.
